The link between oral health in pregnant mothers and the overall health of their unborn child is a relatively recent discovery&lt;br /&gt;&lt;br /&gt;Specifically, the ADA research suggests: “ there may be an association between maternal gum disease and pre-term birth and low birth weight babies. In addition, a recent study says that pregnant women with gum disease may be more likely to develop gestational diabetes.” Without this information, pregnant women have placed dental care low on their list of priorities during pregnancy. These studies may change that&lt;br /&gt;. &lt;br /&gt;If you are planning a pregnancy, schedule a dental checkup for a cleaning. If you are pregnant, continue your regular dental visits. How often do you floss? These are common questions that we ask you when you are sitting in the dental chair. But did you ever stop to think about why we ask you again and again to floss?&lt;br /&gt;&lt;br /&gt;We encourage you to floss for the same reason we encourage you to brush-to reduce the amount of bacteria that live in your mouth. Millions of microscopic bacteria call your mouth “home” because they like to feed off the food particles left on your teeth. &lt;br /&gt;&lt;br /&gt;These unappreciative guests produce acid as a result of their eating frenzies and can get through your enamel, ultimately causing decay. &lt;br /&gt;&lt;br /&gt;Flossing removes any bacteria build-up-also known as plaque-left behind in tiny spaces after brushing. The American Dental Association recommends you floss your teeth once a day.&lt;br /&gt;&lt;br /&gt;So what happens when you don’t floss? Plaque build-up left in between your teeth can eventually harden into a substance called tartar. Tartar cannot be removed by simply brushing your teeth. &lt;br /&gt;&lt;br /&gt;Over time, tartar can produce dangerous bacteria that can lead to a condition called gingivitis. Gingivitis, if left untreated, can then lead to periodontal disease. Periodontal disease is a condition where bacteria get deep into your gums and eventually your bone structure supporting your teeth. They can grow quite large, or they may remain as small bumps. They are also bound to the underlying bone and cannot be moved around with finger pressure. Tori are quite often mistaken by patients for &lt;a href="http://www.drtoneil.com/vizlite.php"&gt;oral cancer&lt;/a&gt;. Very large mandibular toril can become a nuisance since they are covered easily with abraded soft tissue and can become quite sore when eating hard or irritating foods. In situations like this, it is advisable to see a specialist and have them removed. The operation is not especially difficult, and aside from postoperative discomfort, the effects are immediate and quite positive. As you know, we are expanding our office.  You have been most understanding and tolerant of our construction inconveniences and especially the noise.  Sometimes we had to make you wait because one of our treatment rooms lost power, or a computer went down, or a wall had to be knocked down.  We are trying our best to schedule around these inconveniences and keep things running smoothly.  However, things don’t always go the way you want.  &lt;br /&gt;&lt;br /&gt;Nevertheless, we are excited about our expansion.  We will be adding 4 more treatment rooms!  Our Oral Surgeon and our Periodontist will be more accessible to you by not having to work around treatment room availability.  We will also be able to schedule more in-office &lt;a href="http://www.drtoneil.com/teeth_whitening.php"&gt;teeth whitening &lt;/a&gt;appointments and more exam and cleaning appointments. &lt;br /&gt;&lt;br /&gt;In addition, we will be able to better accommodate our sedation patients, which has been growing rapidly. Those of you that have already experienced dental treatment with &lt;a href="http://www.drtoneil.com/sedation.php"&gt;IV or Oral Sedation &lt;/a&gt;appreciate its value: little or no memory of the visit and less after treatment discomfort.  Plus you get more treatment done each visit. We are also adding a new state-of-the-art sterilization room to keep up with instrument sterilization and organization. That is not true. Yep that’s right chewing gum is good for your teeth. If your about and about and don’t have time or access to a toothbrush or toothpaste, the next best alternative is gum. With the one reservation, and that is the sugar issue. If you only chew while the sugar lasts and then throw it away, you will get a sugar attack on the teeth and won’t see the benefits. But if you chew it for longer, it helps clean your teeth. For example, the gum mechanically cleans the surfaces of your teeth as you chew and secondly, possibly the most beneficial, is that it promotes saliva flow.&lt;br /&gt;&lt;br /&gt;Saliva is great for several reasons. For one it has enzymes that digest carbohydrates, it has antibodies that fight tooth decay-causing bacteria. It also contains a buffer that neutralizes the acids that eat away at your teeth.  In addition it also contains minerals that help rebuild parts of your teeth that have been attacked by decay acid. This repair process by your saliva is a very important part of fighting tooth decay. These disorders are often incorrectly called TMJ, for temporomandibular joint.&lt;br /&gt;&lt;br /&gt;People with TMD can experienced severe pain and discomfort that can be temporary or last for many years. By writing about these leaders, their style and approach I hope to provide ideas, tools, tips and techniques that you can use with your organizations and teams to assist in your success. I am sure there are many worthy candidates for this award and it would be impossible to write about them all. Enjoy the articles and please let us know what you think. The January 2009 award goes to &lt;a href="http://www.drtoneil.com/about-dr-oneil.php"&gt;Dr. Thomas O'Neil&lt;/a&gt;. (Dr.O'Neil has been my dentist for 3 years. The reasons for his selection for the first Successful Leader of the Month are:1) What &lt;a href="http://www.drtoneil.com/about-our-team.php"&gt;his staff &lt;/a&gt;says 2.) What I witness when I visit his office.3. What other customers tell me.4. His results demonstrate all aspects of what a quality leader can be. In addition, it is not easy for a dentist to demonstrate effective leadership as they would be prone to "work in the business and not on the business" by getting pulled into the technical aspects of the job and not the leadership aspect. This is not the case with Dr. O'Neil and he and his team make it look easy. I was recently at his office for a new crown. Getting a crown is not the most pleasant experience between the expense and the discomfort. If you do require this procedure you'll want it to be done at Dr. O'Neil's office. It's actually &lt;a href="http://cosmeticdentistryforme.com/"&gt;pleasant&lt;/a&gt; and that is a tough act for a dental office! Dr. O'Neil's entire staff - and I mean all - greet you with a smile. They talk to you and make you feel at ease. I've never seen an attitude other than friendly by anyone - ever!!! The office is impeccably clean and modernly decorated. The waiting room while small, has a friendly, clean and appealing atmosphere with plenty of magazines to read. The receptionist offers a printed card providing several complimentary, comforting beverages. While you are in the chair they offer a diverse variety of music or movies to experience while you wait or are being worked on. The assistant nurses are empowered to do whatever it takes to ensure you are comfortable and they are always checking with you. The nurses are also empowered to do additional work that in some dental offices is only performed by the dentist. At first I was a little concerned by this, however, Dr. O'Neil has trained them so well that they perform these tasks flawlessly. This includes making the &lt;a href="http://www.drtoneil.com/porcelain_crowns.php"&gt;temporary crowns&lt;/a&gt;. This type of empowerment not only builds the skills and abilities of others it also engenders trust, loyalty, and confidence in his team. They all work as a team, communicating what is needed to ensure the customer is handled on time and gets premium treatment. For example, they call out each other's name and ask for help. The response is immediate with no complaining about having to assist a colleague. They update Dr. O'Neil as needed by direct communication in a very professional manner and tone. You can tell it is one office with one goal - to service the patient. How many of our offices run like this? When your customer comes in are they treated as special? Do they see the different roles and responsibilities or is it transparent to the customer as it should be? Notice I've not said one thing about Dr. O'Neil. Why is this? It is because he runs a successful business. He has processes and allows his team to operate in the process. He doesn't have to take the lead role at all times. He creates and empowers leaders. This allows Dr. O'Neil and his team to service more patients and handle volume that builds revenue. In fact most of the time it is his team that leads and runs the office. This makes for great leadership as Dr. O'Neil sets the goals, delegates, trusts, communicates and rewards. I asked Dr. O'Neil if he trains his staff and he does all the time. This includes leadership, team and communication training seminars by a person that specializes in the dental industry. It was after 6:00 p.m. when we finished the temporary crown and I asked the nurse what time she started. She said 7:00a.m. I was truly impressed as you would have thought I was her first patient of the day. She said they are always busy, she loves what she does and times flies by. She walked me to the reception area, pulled up my file, and set up my next appointment. I didn't see Dr. O'Neil and won't until the next appointment which is exactly as planned and how it should be. At 8:30p.m. a call came to my home and it was Dr. O'Neil asking how I was feeling and following up on my well being. Customer follow-up is a tremendous trait for a leader! A new found study In the November2007 issue of the Journal of Periodontology (JOP) discovered that prevention of periodontal diseases may lead to savings on dental and medical care costs. Periodontal disease has been linked to systemic health conditions such as cardiovascular disease, diabetes, and respiratory problems.&lt;br /&gt;Japan examined the effect of periodontal diseases on dental and medical costs in 4,285 patients over a 3.5-year time span. The patient’s age groups range from ages of 40-59. Researchers found that cumulative health care costs over 3.5yrs were 21% higher for patients with severe periodontal disease than patients with no periodontal disease. Severe periodontal disease (periodontitis) involves bone loss and diminished attachment around the teeth. Because of the relationship between the mouth and the rest of the body, treating periodontal disease may be one simple way to decrease total health care costs. If caught early, periodontal diseases can be treated using simple non-surgical techniques which can restore your mouth to a healthy state. The American Dental Association reports that some degree of gum disease affects 75% of adults over the age of thirty five and older. The cause for concern is more than three hundred species of bacteria that live in the mouth. These bacteria's left unaddressed can form colonies between the tooth and gum, leading to gingivitis, an inflamation of the gum and tissues. Gingivitis can develop into periodontitis, a chronic, low grade infection where the infected gums pull away from the teeth and form a growing pocket that allows the infection to spread. If remained unchecked, bacteria can eventually eat away the bone beneath the gums, loosening teeth and threatening tooth loss. Many common dental procedures, like teeth cleanings can cause minor bleeding that allow oral bacteria to enter the bloodstream. For most patients these bacteria are harmless. In the past if you have had a medical history that includes a heart defect, prolapsed valve, rheumatic fever, Endocarditis, Heart surgery, or other problems, we needed to take special precautions to protect you by prescribing appropriate antibiotics for you to take before your dental procedure. In April 2007 the American Heart Association came out with new guidelines for pre-medicating before dental procedures. The NRC panel determined that 10% of kids living in communities where the drinking water is close to the EPA upper limit of safe exposure develop severe dental fluorosis; not only yellow and brown stains on their teeth but also pits in the enamel, increasing the risk of cavities. Rates of dental fluorosis appear to be on the rise. The CDC says the percentage of Americans between ages 6-19 with fluorosis jumped from 23% in the late 1980s to 32% today. As most people desire a healthy glow of white teeth, they need to be just as concerned about the internal structure of their teeth, too. An easy way to monitor such oral health is to regulate and alter your diet.&lt;br /&gt;Eating foods from each of the five food groups is an important tip as it does not restrict you diet to a specific type of food that may contain too&lt;span style="color:#ffff00;"&gt; much &lt;/span&gt;sugars and acids. The best foods from the pyramids are fruits, vegetables, bread, dairy, fish, chicken, beans, and meat( American Dental Association). Snacking between meals should be kept at a minimum because of the amounts of the digestive acids that are released in your mouth after eating. According to the American Dental Association when eating a full meal, your saliva production increases allowing your mouth to be rinsed more thoroughly of these digestive acids. Good snack ideas that promote dental health consist mainly of vegetables and fruits or foods low in cholesterol, fat, and sodium. Drinking water after snaking is also a great plus!&lt;br /&gt;&lt;br /&gt;Vegetarians should also be aware that by consuming meats and poultries their oral health may become endangered. If not substituted with other food sources, a vegetarian diet may lack calcium, vitamin D (important in bone formation and immune system health), proteins and &lt;span style="color:#ffff00;"&gt;other &lt;/span&gt;necessary nutrients. Such nutrients are even more important for young vegetarians ( children and teenagers) as they are still growing. One on a vegetarian diet can eat fruits, vegetables, and grains to made up for missing nutrients. During the training camp, Stanley placed several willing dental offices in what he called, “the hot seat” (where he and audience members would ask the teams questions about their practices). Through refreshing analogies and tried and true patient-care advice, we learned of their successes, expertise and experiences. As a team, we are excited to share what we learned with all patients and to continue with the practices already implemented by Dr. O’Neil in our Tamarac office. I remember thinking, "It can't be true, Patrick Swayze has always been seen as a healthy, vibrant celebrity icon!"  I was in awe that a man who I had seen television and the movie screen throughout my life as a somewhat healthy individual was now dying at the age of 55.  I know they said on the news that he had a good prognosis and that they found the tumor fairly early, but I always remembered hearing bad stories about pancreatic cancer.&lt;br /&gt;&lt;br /&gt;I "googled" pancreatic cancer online to see what it was all about.  In an article written by &lt;a href="http://www.cancer.gov/"&gt;the National Cancer Institute&lt;/a&gt;, they say that "only 4% of people diagnosed with pancreatic cancer will survive for more than 5 years after they have been diagnosed."  The rest will not even live that long.  That was very scary to me!&lt;br /&gt;&lt;br /&gt;I proceeded to look at the signs and symptoms out of curiosity.  Much to my surprise, being in the dental field, gingivitis and periodontal disease were on the list!  I knew &lt;a href="http://drtoneil.com/general_dentistry.php"&gt;oral health&lt;/a&gt; was related to heart disease, but pancreatic cancer, too?&lt;br /&gt;&lt;br /&gt;The above story just reconfirms how important your oral health really is.  Not just with your mouth and heart, but throughout your body. Plaque is the soft, sticky layer of bacteria, which is constantly forming on the teeth.  Usually it is invisible to the naked eye, but when a person is not brushing adequately, it can build up to where it appears to be a thick whitish coating on the teeth at the gum line.  If not removed, it can lead to &lt;a href="http://www.drtoneil.com/general_dentistry.php"&gt;gum disease&lt;/a&gt;.&lt;br /&gt;&lt;br /&gt;Another potential problem, which is easy to spot, is &lt;a href="http://www.drtoneil.com/extreme_makeover.php"&gt;missing teeth&lt;/a&gt;.  Many patients assume that if they are still able to eat, they are O.K.&lt;br /&gt;&lt;br /&gt;But very often, losing just one tooth can lead to the loss of support and teeth begin to drift into the empty space, causing a change in the bit.  It also forces the remaining teeth to carry an additional load, sometimes past their ability to adapt. Our patients always remark about  the thoughtfulness placed on the added comforts in the office, especially the variety filled platter of cookies Chris asks us to place in the waiting room. She would get a kick out of the  reactions and conversations we hear daily. From our new female patients, she would hear,  “Cookies,  how nice, how thoughtful!”  The kids excitedly say, “Cookies! May I have some please?”  The men usually ask in a reserved tone, “ Cookies? In a dentist waiting room? Then, once they think no one is watching, they sneak a couple, maybe three. The next thing noticed is that they have claimed the closest chair to the platter. She would hear which was their favorite and tell each other why it’s  the best one  on the plate.  Whomever the  cookie connoisseur may be, established patient or new, they are now relaxing with a beverage from the drink menu, perhaps for a moment forgetting they are at their dentist’s office. &lt;br /&gt;Many little “comforts” are offered to our patients while  they are at the office.  Whether it be a cup of hot tea, a cool drink, a warm neck pillow, their favorite music and even a complimentary pre pasted toothbrush {for freshening up after the cookies} available upon request in the patient restroom.&lt;br /&gt; At Dr. O’Neil’s dental office in Tamarac, Florida,  our patient’s comfort is top priority. It might surprise you to learn that it is the &lt;a href="http://drtoneil.com/general_dentistry.php"&gt;bacteria&lt;/a&gt; that cause periodontal disease.  Although cardiovascular disease has previously been linked with periodontal disease, Ann Progulske-Fox, PhD, professor of oral biology at the University Of Florida College Of Dentistry, conducted a study that showed the presence of live oral &lt;a href="http://drtoneil.com/general_dentistry.php"&gt;bacteria&lt;/a&gt; in blood vessel tissue.  Her findings were published last year in the journal of Atherosclerosis, Thrombosis and Vascular Biology.  &lt;br /&gt;Studies have shown a direct correlation between the amount of periodontal &lt;a href="http://drtoneil.com/general_dentistry.php"&gt;bacteria&lt;/a&gt; present in the mouth and the formation of blockages in the carotid artery.  These findings prove that periodontal disease has far more serious implications than the possibility of losing teeth.  It can become a life threatening condition.&lt;br /&gt;&lt;br /&gt;Progulske-Fox is now conducting additional research to determine whether periodontal infection can also contribute to a weakening in the blood vessels that leads to brain aneurysms. There are different reasons why teeth may crack. One is that they endure a tremendous amount of pressure from biting and chewing. Another reason is the heavy stresses of clenching and grinding. Decay, large silver fillings, chewing on ice or popcorn can also result in cracked teeth. To diagnose cracked tooth syndrome we will perform a thorough exam. If the crack affects only the outer enamel layer of your tooth it can be restored with a crown or onlay. If it affects the underlying dentin or pulp layers, the tooth may need root canal therapy before we can place a crown. In rare cases, if the crack extends all the way through the tooth and under the jawbone the tooth cannot be restored and must be extracted. Its important to evaluate and treat cracks as soon as possible because they can grow quickly. To be totally successful with sedation dentistry you must understand the emotional state and have a deeper understanding of medicatons and drug-drug interactions.  Dr. O'Neil purchased a software that we can enter the medications patients are currently taking, and it will let us know of any drug interactions as well as detailed information about the medications.&lt;br /&gt;     Through this sedation seminar we learned about protocols of different diseases, and medications that people are commonly taking.  We also learned more information on how to make a patient feel more comfortable and at ease at the dental office.&lt;br /&gt;     We really enjoyed our seminar and will be attending many more to keep up with our education. We are very proud of our Clinical team because they were the most knowledgeable team at the lectures. Once the unit is turned off the Nitrous is off and you are free to go about your daily business (no real downtime after procedure with this). You are able to drive to and from your appointment and you are also able to eat or drink prior.&lt;br /&gt;&lt;br /&gt;-Ativan- a pill that is taken 1 hr prior to appointment to relax you and relieve anxiety. You will need a driver to take you to and from your appointment and you will probably want to stay home and rest after the procedure, due to the fact that Ativan will make you a little drowsy.&lt;br /&gt;&lt;br /&gt;-Triazolam-AKA: Halcion- a pill that is taken 1 hour prior to appointment, must be taken on an empty stomach, meaning no food or drink for 12 hours prior. You will need a driver to take you to and from your appointment and you will probably want to stay home and rest that day. What most patients love about Triazolam is the benefit of almost 100% amnesia, therefore, they go home having no memory of the entire appointment!!&lt;br /&gt;&lt;br /&gt;-IV Sedation- this is a combination of different medications administered intravenously while you are in the office. You must have an empty stomach , this means absolutely no food or drink 12 hours prior to appointment. You will need a driver to take you to and from your appointment and you will probably want to stay home and rest that day. This statement may be true for some, but not for all. I have seen the difference between the two over and over again during the course of my practice. Recent studies have shown that power toothbrushes decrease the amount of plaque build up, &lt;a href="http://www.drtoneil.com/general_dentistry.php"&gt;pocket depth&lt;/a&gt;, bleeding, gingivitis, stain accumulation and therefore improve health in general. According to some periodontists when using an electric brush, the bristle gets into the sulcus as far as 3.5mm where as when using manual brush it goes down only to .5mm in the sulcus.&lt;br /&gt;&lt;br /&gt;I saw a female patient in her 60's a month ago with pocket depth of 4 to 5mm, inflammation of the gingiva, moderate bleeding upon probing and some redness. I recommended that she comes in for scaling and root planning and that she uses the Sonicare toothbrush which is my favorite power toothbrush: especially since I have seen good results over the years with my patients. The patient came back for Scaling and Root Planning two weeks later after her initial visit for her treatment and when I looked in her mouth, her gum tissue was pink, there was a reduction in pocket depth and so on. It is normal for the gum tissue around your tooth to be a little sore when the anesthesia wears off. This will disappear by tomorrow. Do your best to chew on the opposite side until your next appointment. Chewing sticky foods can sometimes dislodge temporary restorations. It is not uncommon to have the following reactions or problems with your temporary.&lt;br /&gt;&lt;br /&gt;Kathy from Ft Lauderdale had some questions about her temporary crown. Here is a list of some concerns that we hear from our patients and our responses to them.&lt;br /&gt;&lt;br /&gt;&lt;br /&gt;It feels rough&lt;br /&gt;Please call us if it is making the tongue sensitive.&lt;br /&gt;&lt;br /&gt;It came off&lt;br /&gt;This is probably the most common problem. Temporary restorations are supposed to come off easily and some foods will help in this. Stay away from sticky foods or gum. It’s important for it to be there. Often, you can place it back with little trouble. If you are unable to replace it, call us and we’ll take care of it for you.&lt;br /&gt;&lt;br /&gt;It feels high&lt;br /&gt;Come in so that it can be adjusted. A high temporary can cause painful bruising to the tooth.&lt;br /&gt;&lt;br /&gt;It’s not how I want the final restoration to look&lt;br /&gt;Don’t worry, the final restoration will look like your natural tooth.&lt;br /&gt;&lt;br /&gt;It is sensitive&lt;br /&gt;The temporary will not always fit as well as the final restoration, so there may be some sensitivity to hot, cold, and chewing. If it is too uncomfortable, please call.&lt;br /&gt;&lt;br /&gt;The temporary is very important. It protects the exposed dentin, prevents food from gathering in the preparation and prevents the prepared tooth from shifting and moving. The temporary helps in making seating of the final restoration trouble free. Use your toothbrush to clean the temporary as you normally do your teeth, but don’t floss around the temporary, as you will dislodge it.&lt;br /&gt;&lt;br /&gt;If you have any questions concerning the temporary, please feel free to call. This disease is silently affecting 50 million Americans (www.orapharma.com). It is also known as periodontal disease, an infection of the connective tissue and bone surrounding the teeth, if left untreated the disease may lead to tooth loss. It is a disease of neglect.&lt;br /&gt;&lt;br /&gt;The main cause of gum disease is plaque, a soft, sticky colorless film of bacteria above the gum line. When the gums become red, swollen and start bleeding, it is vital that the patient seeks professional help before the disease progress to a non reversible stage. Regular dental prophylaxis and proper home care is essential for a healthy gum and smile. &lt;br /&gt;&lt;br /&gt;If the plaque is not dislodged from between the teeth and the gum, the body’s immune system will fight the disease; therefore cure the symptom of gum disease discussed above. The patient in turn will assume that the problem has been resolved when in fact the disease is progressing slowly to periodontitis, a more serious damaging stage of gingivitis.&lt;br /&gt;&lt;br /&gt;Another important factor that is very important for everyone to be aware of is that periodontal disease is hereditary. There is an increasing evidence that the disease is contagious, the causative bacteria can be passed from parent to child or from lover to lover simply by kissing (www.afdd.com). It is possible to have periodontal disease and have no warning signs; that’s why it is important to get regular dental check ups and periodontal examination. Good oral hygiene at home is essential to help keep the disease from becoming more serious or recurring after treatment. There are a lot more causes associated with gum disease: stress, certain medications, diabetes, poor dental restorations, dry mouth and so on.
